[Discussion] Custom paper sizes?

This morning I decided to use my new toy, OpenOffice, to address a
Father's Day card. The card came with an envelope that is about 5" x 8".
Formatting a page to a custom size in OO is easy enough, but getting it to
print in the proper place on such an envelope is not.
My printer is a Lexmark Optra M410. I'm using the postscript driver
30.827. There is no paper size of 5 x 8 available as an option in the
printer's forms menu, although DL Envelope is sort of close. Even after I
created a printer object of DL Envelope size and formatted the page to
that size, I got a forms mismatch when printing it.
How does one add new forms to the printer driver when "Custom" is not an
available form in the list? Is there an easy way to print on paper sizes
other than the supplied ones? I can use a standard size but manipulate
the margins or the position of the frames to force the type to print in
more-or-less the correct place, but there must be a more elegant solution.
Yes, I also have a pen, but that's so 19th Century. :-)
